What to Expect During Your Visit
We understand that visiting a new healthcare provider can feel uncertain, which is why we’ve designed our process to be comfortable and informative from the moment you walk through our doors. Our goal is to make your first experience with us as smooth as possible while gathering the information we need to provide you with the best possible care.
Before Your Appointment
Once you have scheduled your new patient visit, our office will email new patient forms to the email address that was provided. It is important to please fill out forms skipping any areas which may not apply to your visit. These forms will be directly entered into our system.
What to Bring
- For our Physical Therapy patients with Medicare, please bring valid photo ID as well as your medicare & supplemental insurance cards for reimbursement purposes.
- List of current medications and supplements
- Any recent imaging studies (X-rays, MRI, CT scans)
- Previous medical records related to your condition
- Comfortable clothing that allows for easy movement during examination
Your Treatment Journey
Personalized Care Plans Based on your examination and consultation, we’ll develop a treatment plan tailored specifically to your condition and lifestyle. We believe in educating our patients about their condition and involving you in treatment decisions every step of the way.
Treatment Frequency The frequency and duration of your treatment will depend on several factors including the nature of your condition, how long you’ve been experiencing symptoms, and your individual response to care. We’ll discuss realistic expectations and goals during your initial consultation.
Progress Monitoring We regularly assess your progress and adjust your treatment plan as needed. Open communication about how you’re feeling and responding to treatment helps us provide the most effective care possible.
